Yup, one should play the way that he/she can proudle wear the name of his/her character.

Your question was answered. No. You can not EVER change the name of a character once it has been created.

If that name format is a requirement for this guild I would first suggest finding a more friendly guild. Other than that you could create a new character with that name format and have the guild invite that character. Then you can continue to play as any of your characters and you will still be "known" to the guild under the one with the name format.
